When you hear lower back pain is associated with the TCM concept of kidney, do you think that is because the Western Kidney is actually in pain from malfunction or disease, or does the TCM kidney somehow affect the muscles of the lower back?

It usually has nothing to do with the western Kidneys, it is due to a lack or blockage of qi and blood, causing weakness and stagnation. There are many other reasons for low back pain in TCM, not just deficient Kidney Yang.
