If a post-menopausal client is on statins, at what, if any, point would you be concerned about her cholesterol going so low that it impacted her hormone balance?

Wendy said: I don't want the LDL much below 90. If it's all the way down to 60 or less, it would definitely impact ANY man OR woman's ability to make any steroid hormone.

Wendy. What about if a woman's diet is so low fat that her total cholesterol is 130 and LDL 70? I am concerned about borderline hypolipidemia which would negatively impact hormone production. In this case I would think about recommending more healthy fats (ghee, fatty small fish, salmon, etc). Also, looking at the literature, there can be an increased risk of intracranial hemhorrage up to 5X with hypolipidemia (this study excluded patients on statins). The mechanism is thought to be loss of vascular integrity increasing the likelihood of a hemhorrage.

Wendy said: I absolutely agree that if her DIET makes her lipids this low, she needs a lot more healthy fats. Ghee, olives, seeds, nuts, etc. LDL should be under 100 but not a lot under 100. Just remind her that all steroid hormones come from cholesterol so if her total cholesterol and LDL cholesterol specifically is TOO low, she can't make hormones.
